Very Good book on Root Cause Analysis.......2007-02-21

This text gets right to the point about root cause analysis and the various analytical tools that are used in the process. It is a great supplement to the material I've review on the subject for class work. I highly recommend this text for anyone who is studying the subject for accident investigation or process improvement.

Root Cause?? More Like Finding a Scapegoat!!.......2005-03-05

Accidents happen because of an amalgamation of errors, not simply because of one person's "human error". Design faults, management pressures, poor training, long shifts, poor device interfaces, insufficent safety guards, poor maintenance, and many other issues come together to afford an accident that would not have caused that accident alone. Finding root cause enables blame to be placed and make the rest of the system feel "safe" while latent problems still linger. Safety is created by fostering a culture that sees risks for what they are and works to minimize those risks (better training, better safety guards, lessening worker fatigue) and NOT by placing blame on one "root cause".

Very good introduction to root cause analysis........2004-01-22

This book does an excellent job of quickly covering conventional root cause analysis. With an emphasis on documentation, the author takes you through a series of steps that include defining the problem, methods for collecting data and gathering information, analysis of the problem, and ultimately developing corrective action. This book moves very fast, reading more like a slide-show presentation than a book, yet the author is able to communicate a lot of good information using this minimalist approach. Rather than detailing numerous analysis methods, the author keeps it simple by sticking with a fixed path to problem solving. I found the "Pitfalls of ..." sections after each topic very insightful. In addition, the collection of forms, tables, and checklists included in the book (great appendix) are excellent. Don't look for case studies here, this is a straight forward how-to book

Unique approach to a traditionally reactive technique.......2001-02-13

My background is information systems service delivery. I stumbled across this gem when I was searching for resources on how to structure outage investigations for applications that did not meet service level objectives.

Prior to reading this book I viewed root cause analysis as a reactive tool to be used to investigate the causes of failures. The approach taken by the author is to also use this technique as a proactive tool in a manner similar to failure mode effects and criticality analysis (FMECA).

Here is a summary of what how this book approaches root cause analysis: it begins with a section on defining problems and collecting data. This approach allows you to take a proactive approach, especially if you are in an environment that uses statistical process controls to measure process performance. This is augmented by a chapters on task analysis and event and casual factor charting. This is where the author's approach begins to resemble FMECA techniques, which are proactive versus reactive methods of addressing problems and risks. This is followed by chapters on interviewing and reporting.

I particularily liked the chapter on interviewing techniques, which added a practical dimension root cause analysis. I also liked the way the author used hints throughout the book to reinforce methods.

The rest of the brief 135-page book is devoted to forms, worksheets and checklists that significantly add to the value or the book. I would have liked an accompanying diskette with this material in electronic format, but the lack of it does not detract from the book in any way.

If you want to use root cause analysis as a proactive tool in conntection with a continuous improvement initiative I recommend that you also consider Understanding Variation by Donald Wheeler. If you are seeking a more proactive approach to preventing problems in the first place you might consider augmenting Root Cause Analysis Handbook with Failure Mode and Effect Analysis by D. H. Stamatis.

Great book about a methodology I use and know works!.......1999-03-20

Provides a clear, step by step methodology, with techniques, for problem or error analysis and identifying a solution. This is an exceptionally practical work offering a methodology for problem solving that is attractive; one which I subscribe to and use. Recommended.

GREAT learning tool!.......2002-09-02

For the last six years, I have been teaching Social Statistics to a wide variety of non-math majors (i.e., Nursing, Social Work, Sociology, Criminal Justice, Education, and Political Science). During the summer of 2002, I had an opportunity to teach a section of statistics exclusively to Nursing majors. Nurses are GREAT students. They seemed to have an intense interest in the application of statistics to nursing practice and evaluation. In addition, I learned that our local nurses are required to employ control charts to file a wide variety of reports to the hospital administration. In an effort to teach relevant material, I searched amazon.com for "Control Charts" and discovered Kelley's outstanding book.

I can make a couple of points that may prove helpful to any reader of this review. First, Kelley is a wonderful writer. Making statistics fun, interesting, and relevant is a Herculean task. Kelley accomplishes this objective. Second, anyone who can get a C as a final grade in an introduction to statistics course will be able to understand and use control charts after reading Kelley's book. She offers GREAT examples and gives self-grading quizzes to facilitate the learning process.

Every nurse with whom I shared this book, seemed to appreciate it. Several of them actually purchased it. Thus, if you need information about control charts in a health care setting, Kelley is the best (and perhaps only) book on the market.

I found the book to be very easy, straightforward, and exactly what I needed. I was totally unfamiliar with control charts, how to set them up, and what they did. This book presents simple examples and is easy to understand.

Do not agree with some of the "tools", especially "steering". Do not think that an intermediate skier (as myself) can benefit from the "tools" introduced in this book. I did learn something new in this book, though not much.

Yes, this book contains much information about the different components of skiing, ie., balance, weight transfer, pressure control, etc. etc. It also contains a wealth of exercises/drills that allows one to improve every aspect of how they ski...

Unfortunately, there are absolutely minimal pictures (cartoon drawings at best) to explain those concepts, and those exercises/drills..!! Yes, there are pictures - but they are mostly for posterity and not for supporting the words contained in the book.

For a subject like "Skiing," diagrams are a tremendous help, and this book has much to be desired on that area...

I Have Seen The Light, One Little Step At A Time.......2007-03-19

Woah. Problem with skiing instructors (and dance instructors too) is that they put too much into each hour lesson. With this book I was able to read it at bedtime, take it skiing and read it at lunch. I started with jumping around reading part and decided to take the author's advice and get my "tool set" in order so I started at the beginning. Whoa! 2 ski days this weekend and I feel like I've made incredible progress. (Trying to ski the black diamonds). Each chapter is full, but I would come away with one item at a time that I would remember on the slope.

If you are picking up speed that you don't want and then compulsively traversing the hill until you are stuck on your uphill ski, this is because you haven't completed your turn. That's me!! An expert skier is always turning, loading and unloading and using the C-shaped turn to keep their speed down. All of a sudden, the black was no longer scary! Going slow and falling on steeps is very harmless stuff!

Keep your stance very straight on shape skiis. People complain about burning quads and weak legs. It's because you are leaning into your boots and using your quads to take the energy in a turn. You should be using your bone structure. That's me!!! I was going to buy women's skiis because of this problem. Instead, I tightened my boots as stiff as I could so they wouldn't flex forward and I concentrated on skiing with a straigher leg, and guess what? I could ski the whole run and not get tired. (there are times when you need to get down, but a lot less than I thought).

I normally wouldn't think twice about my arms and pole planting. Boy, after reading that chapter last night, I decided to spend the whole day on the slope concentrating on my arms: shoulder width plus a little, don't swing your arms, swing your hands straight forward in relation to the skiis, and KEEP YOUR HANDS(BOTH) VISIBLE IN YOUR PERIPHERAL VISION! I wasn't doing this. You don't have to keep your hands way out in front or really wide like straight skiis demand. And the biggest thing about pole planting? You are always swinging the pole! Your pole is either coming from behind or planting and then falling away. This is pretty no brainer on tight S-turns(the secret behind steep skiing) but on the long GS turns, I was waiting and then at the last minute swinging my pole forward and planting. Brad says this causes your skiing to be erratic and low and behold, I saw a change immediately! All of a sudden it is all related into one fluid dance.

The expert skier is always turning, making us of the dynamics, turning complete "C" turns to maintain speed on the steep, and swinging their poles into plant (sometimes you use more of a pole, sometimes you don't). Everything falls into place. My turns started matching my pole rythm and/or vice versa and I started feeling like I was really skiing.

Get the latest edition, I believe it is the 2nd Edition because it has been updated for shape skiis.

What I appreciate the most about this book is that it covers everything...in detail! Understanding then incorporating the subtle adjustments to stance, conditions, equipment, etc. allows the refinements to become part of better form. That translates into more fun skiing. There are a lot of paperbacks out there. The two "must have" books are this one, and Witherell's "Athletic Skier".

I've bought three books about skiing recently and this is the best. I'm an aspiring expert skier. This book is really helping me reach my goals. The book has a chapter each on the repertoire of fundamental skills like stance, steering, outside ski dominance, edging, pressure control, etc. The book would great for that alone. But the book in later chapters explains how to use these tools to carve, ski powder, crud, bumps, steeps and trees. I'm reading this book again and again. It's the bible for people wanting to be and expert skier.

If you don't own a gazeteer for your state I would suggest buying one. It is nice to be able to take the path less traveled and these show all of the county roads, etc. It also helps if something has closed an interstate and you need to find another way.

A must-have for any Ohio fisherman.......2002-08-17

I like to scout out small lakes off the beaten path, and this Atlas has proven invaluable to me. Picture this: If you cut out all the pages and re-assembled them into a one-piece map it would be as roughly as big as the average front yard! Hardly anything is left out, including many small farm pond sized bodies of water. My best discovery to date was a tiny 'lake' South of Columbus, Ohio, with a picture of a fish on it. I made the short trip and proceeded to catch a 6 pound Bass out of it- unheard of in the more heavily populated lakes around Columbus.

In Horsefeathers' Mystery Scoop is boarding three race horses Jackson, Storm, and Melly. Scoop falls in love with Melly. Someone is trying to hurt the horses, one night Scoop finds one horse bleeding. Scoop finds her brother BC playing with a burnt stick and thinks he has been playing with matches. The end is very sad and I was surprised when I found out who was trying to hurt the horses.
